The Hudson Valley Social Care Network (SCN), led by Hudson Valley Care Coalition, is building a connected system of care that helps Medicaid Members access vital supports for food and nutrition resources, housing assistance, and transportation to those supports. It all starts with screening and navigation provided by partners like you, who are helping at-risk populations gain one-stop access to health-related social services.
This toolkit was created for our partners—community-based organizations, health care providers, and service agencies—to help you share information about the SCN and screen Medicaid members.
