Since 2012, Hudson Valley Care Coalition (HVCC), a 501c3 nonprofit, has been the Hudson Valley’s trusted connector for clinical, behavioral, and social care. We convene FQHCs, behavioral health providers, payers, hospital systems, county governments and community organizations to deliver coordinated, person-centered, value-based care for Medicaid members. Our work covers the spectrum from prevention to complex care. As the Lead Health Home, Behavioral Health Independent Practice Association and Lead Social Care Networks, HVCC manages an integrated network of 100+ partners across seven Hudson Valley counties.
By aligning community-based support with healthcare systems and managed care goals, HVCC helps partners improve outcomes, get connected to resources, reduce avoidable ER visits and hospital admissions, and meet the demands of value-based care. We provide the infrastructure, technology, and partnerships that give every Medicaid member a clear path to whole-person health.
HVCC delivers measurable results through the organization’s Health Home program—serving more than 5,000 high-need Medicaid members with complex medical and social challenges.
