Am I Eligible for SCN Services?

Any Medicaid member is eligible for screening by one of our provider partners or by completing a self-screener online. Medicaid Managed Care Members with certain health conditions and social needs may be eligible for one-time or temporary services as part of the Social Care Network program, in addition to existing benefits and services already available in the community. Find additional details on eligibility in our FAQs below.

The self-screening form is simple and confidential. Once submitted, you will receive a call from one of our partner screening organizations within 5 business days.

Even if you don’t meet all the criteria, you may still be connected to similar services through other state, federal, or local programs. A quick, free screening will help determine the best options for you.

How do I know if I’m eligible for Social Care Network services?

  • You’re enrolled in Medicaid Managed Care
  • You have one or more unmet health-related social needs (such as lack of healthy food, safe housing, or reliable transportation)
  • You’re a part of an Enhanced Service Population, which can include children and adults, members with behavioral health conditions, pregnant persons, and other Medicaid populations who would benefit from additional support.

Call us to speak with a care navigator at 914-215-5292 or complete the Self-Screening Form.

If you have a concern with your HVCC Social Care Network experience, complete the Member Complaint Form or call us at 914-215-5292.

Participating in both programs won’t affect your Health Home enrollment or your Medicaid coverage.
