As the heat index continues to rise, HVC is preparing for the first time ever One-Day Symposium, “Shaping the Future: It Starts With Us”, honoring and thanking our care managers for their incredible work, by providing incredible speakers to bring us the ins and outs of health care management. Speakers include Dr. Amy Boutwell, nationally known for her work in Transitions of Care, and Peg Elmer from the Department of Health, sharing her knowledge from her experience as the Director of Health Homes. This symposium will take place on Friday October 12, 2018 at the DoubleTree Hotel in Tarrytown, NY.
