Hudson Valley Care and Coordinated Behavioral Health Services Unite to Better Integrate Primary Care, Behavioral Health and Social Care in the Hudson Valley

This Transformative Alliance Will Coordinate Health and Social Care to Improve Access to Services, Drive Better Whole Health Outcomes, and Lower Costs of Care.

Hudson Valley, NY March 14, 2024 – Hudson Valley Care (HVC), the leading health home in the Hudson Valley and Coordinated Behavioral Health Services (CBHS), the region’s most comprehensive behavioral health independent practice association today announced they have united to form a new joint venture, Hudson Valley Care Coalition (HVCC). HVCC will become a regional integrated ecosystem of healthcare, through a combined network of more than 60 agencies including FQHCs, Hospitals, health plans and community-based organizations providing medical, behavioral health, developmental disability, and social care services. The HVCC network will expand the Hudson Valley community’s access to primary care, behavioral health care, care management, and social care services – driving better whole health outcomes for the region’s most marginalized individuals.

With more than a decade of experience integrating primary care, behavioral health, and social services, and expertise in value-based payment arrangements, HVCC is well-positioned to establish our integrated network of diverse providers as the region’s Social Care Network given the organization’s unique alignment with New York State’s 1115 Health Equity Reform Waiver.

Given the impactful collaboration we’ve already undertaken, formalizing our partnership will establish a unified network of health and social care providers, strategically positioned to enhance service delivery for our communities.
